How to Remove Form/Waiver Requirement from Appointments

If you have an appointment type that requires a form or waiver but you want to make it optional or remove it entirely, follow these steps.

Steps to Remove the Form Requirement

Step 1: Go to Appointment Types

  1. Go to Calendar in the left sidebar
  2. Click on Scheduling
  3. Select the provider whose appointment type you want to edit
  4. Find the appointment type in the list

Step 2: Edit the Appointment Type

  1. Click on the appointment type to edit it
  2. Find the Form field or dropdown
  3. Either:
    • Select "None" or clear the form selection to remove the requirement entirely
    • Or uncheck any "required" toggle if you want to keep the form as optional

Step 3: Save Changes

  1. Click Save to apply your changes
  2. The appointment type will no longer require the form

What This Affects

  • New bookings: Customers booking this appointment type will no longer be prompted to fill out the form
  • Existing bookings: Already-booked appointments are not affected
  • Form data: Previously collected form responses remain in the system

When to Keep Forms

Consider keeping forms for appointment types that need:

  • Liability waivers (for safety/legal reasons)
  • Health questionnaires (for fitness or medical services)
  • Intake forms (for collecting essential client information)

Tip

If you want different requirements for different appointment types, you can:

  • Create separate appointment types with and without forms
  • Use one form for initial consultations and no form for follow-up sessions
